Process
for free & reduced
school meals
.
1. Obtain a household application for free and reduced school meals.
You may obtain an electronic copy here:

Download and print the application in English.

You can find the application at your school district’s website:

You may pick up a paper application from the following location:

2. Fill out the application completely and accurately.
u
Please be sure to list the student(s) name, current school, and any other required information as stated on the application.
u
Please list the name and income level of each member of your household.
u
Please fill out all other requested information.
u
Sign and date the application.
IMPORTANT!
u
An application cannot be approved until all requested fields have been completed and signed by an adult household member.
u
If your household receives benefits from the Supplemental Nutrition Assistance Program (SNAP), then your child automatically qualifies for free school meals. Please contact your school to determine if you are required to fill out an application.
3. Return or mail the application back to your school district as indicated below.
u
Please return finish application to Office of Food Service Dept. All last year status will apply until September 16, 2011.
