for free & reduced
school meals
.
1. To obtain a household application for free and reduced school meals:

Please enter in the exact Meal Sense URL for your school in your web browser and it will take you to your schools Meal Sense website. This should be in the form of http://www.mealsense.org/SCHOOLNAME where SCHOOLNAME is the extension used to identify your school district. If you saw this URL in an ad or a poster, please refer to it.

You can also visit your school district or call them to obtain an application
2. Fill out the application completely and accurately.

Please be sure to list the student(s) name, current school, and any other required information as stated on the application.
u
Please list the name and income level of each member of your household.
u
Please fill out all other requested information.
u
Sign and date the application.
IMPORTANT!

An application cannot be approved until all requested fields have been completed and signed by an adult household member.

If your household receives benefits from the Supplemental Nutrition Assistance Program (SNAP), then your child automatically qualifies for free school meals. Please contact your school to determine if you are required to fill out an application.
3. Return or mail the application back to your school district.
